Basic Legal Requirements
- Must be a resident of New York.
- Must be 21 years of age or above, unless given consent of parents or guardian.
Proof of Qualifying Conditions
- Must be suffering from or undergoing treatment for any of the qualifying conditions.
- Receive certification from a registered doctor for the use of medical marijuana
Complete the Paperwork
- Must complete application form and other documents to be submitted to the DOH
- Patient must pay a $50 fee, which can be reduced or waived if experiencing financial hardship
